For Windows [main]:
WAMP - much better than XAMPP, easier and fast to use
Notepad++ - use it constantly
PHP Designer - hardly ever touched it

For Mac [my Hackintosh, also used often]:
MAMP Pro - over kill, i know.
Panic Coda - best thing happened to me in a long time

For Linux-Fedora on X [rarely used]:
LAMP - yes, i'm a lazy bastard.
VIm and Kate - VIm is an over kill, Kate doesn't really cut it... That's why I don't use Linux.

I use Dreamweaver, but more for its autocomplete and FTP file mangement (which is amazingly easy to use, especially with CS3) than its WYSYWYG functionality (which is crap). I usually blank the default layout and write pages from scratch.

I edit all source code with Emacs, its simple and the most usefull editor.

If working fast remotely in PHP: nano/vi
If working remotely on a ASP/.NET server: Notepad
If working locally in .NET: Visual Studio Enterprise
