What Makes a HDD go bad??

I have a samsung 250gb HDD for a laptop i believe it has gone bad, I am trying to re-install the OS on it but it is not working, I have re-formatted it and started running the OS install but keep coming up with different errors upon start up i get boot manager error clfs.sys Status: 0xc00000e9 file is either missing or corrupt so i try and re-install it by re-formatting it again and then install and then i get errors at boot manager windows\system32\ntkrnlpa status: 0xc00000e9 file is missing or corrupt not sure what is going on with this install.If any of you got any ideas or suggestions for trying to figure out what is wrong please help

Also i have popped in the OS CD and used command prompt for chkdsk found some indexing errors so i ran a chkdsk/r and it ran through and yet i am still coming up with the missing or corrupt files, kinda weird i think i also ran the windows CD hardware diagnostic software and it is coming back with bad HDD but i would also care for input on the subject besides mine and googles
